Insurance Benefits
We accept most local and many national insurance plans. Before your first appointment, we recommend contacting your insurance provider to verify that our practice is in your plan's network, confirm your copay, coinsurance, or deductible responsibilities, and determine whether a referral from your primary care provider is required. The customer service phone number is typically listed on the back of your insurance card.
Understanding your insurance benefits is your responsibility. Please review your plan carefully to ensure you are aware of any network restrictions, referral requirements, coverage limitations, and your financial responsibilities for services received.
Diagnostic Procedures
We want you to be aware that when a referral to a specialist is needed, additional diagnostic tests or procedures may be required to properly evaluate and guide treatment. These services are often performed during the specialist visit as part of the standard evaluation. Depending on your insurance plan, these procedures may be billed separately from your office visit copay and could be subject to your deductible or coinsurance responsibilities.
Managed Care Referrals
Most patients do not need a referral from their primary care physician.
However, if you are enrolled in an Medi-Cal, HMO or a similar insurance plan, a referral may be required before you can see a specialist. If we have not received the necessary referral before your appointment, your visit may need to be rescheduled.
It is your responsibility to understand whether your plan requires a referral and to obtain it in advance if needed. Without a required referral, you may be responsible for the full cost of your visit.
Financial
If you provide complete and accurate insurance information, we will submit claims directly to your insurance company and receive payment for covered services. Based on your insurance plan, you may be responsible for copayments, coinsurance, or deductible amounts, which are due at the time services are provided.
We accept cash, checks, and major credit cards including Visa, MasterCard, and Discover. Payment plans may be arranged individually through our Billing Department.

Self-Pay Patients
Our office requires patients without insurance to pay in cash or with a credit card on the date of service. If you have any questions or concerns, please contact our office
Health Plan Participation
We participate with most major health insurance plans in the area. We recommend contacting your insurance provider to confirm that our practice is included in your specific network. If you need your insurer’s phone number, it can typically be found on your insurance card.
Please also review your insurance policy to determine whether a referral from your primary care physician is required.
